Signing up for the Employee Advantage Card is easy. Simply fill out the New Member Enrollment Form on the back of this flyer and return it to your employer or Lifetime Health & Wellness.

After enrollment you will receive your Employee Benefit Kit including your personalized Employee Advantage Card, which entitles you to unsurpassed benefits.
